Application and Parent/Student Information

Request for Records Transfer Form and the following:

• Birth Certificate
• Proof of Immunization or Statement of Exemption
• Standardized Test Scores that have been completed in the last two years 
• Report Card
• High School Transcript, if applicable

Depending on current Standardized Test Score, additional testing may be required to determine if your child is a good fit for RCA.
